MODERN APPROACH IN THE TREATMENT OF CHOLEDOCHOLITHIASIS
Abstract
Currently, choledocholithiasis remains one of the menacing complications of cholelithiasis and meets from 8 to 30% in the structure of the disease. The “gold standard” in the treatment of choledocholithiasis is considered to be minimally invasive, video endoscopic surgical interventions. The priority direction is endoscopic papillosphincterotomy, balloon papillodilation in combination with lithotripsy (contact, laser) and extraction of stones, laparoscopic choledocholithotomy. Among the minimally invasive techniques in the treatment of choledocholithiasis, the reasons preventing the full-fledged operative intervention are often traced, such as multiple and major choledocholithiasis, changes in the biliopancreatoduodenal zone as a result of chronic inflammation and surgical interventions. The question remains in the choice of surgical intervention in this complex category of patients.
