Influence of enteral feeding administration with an elementary composition on a frequency of complications associated with an enteral feeding in children in icu

Abstract

The use of products with elemental composition for starting enteral feeding in ICU can lead to a lowering number of failures associated with poor feeding tolerance. For testing of this hypothesis the trial has been performed with a goal to study the influence of elemental nutrition on a frequency of complications associated with enteral feeding in pediatrics. In this dynamic trial with a method of paired design were included 50 patients of two multi-field intensive care units. The patients included in the index group were supplied with an enteral feeding with an elemental composition. The observational group was supplied with a monomeric nutrition composed of 5% glucose solution and electrolytes. The cancellation of feeding in an observational group has been required more often OR 1.9 (1,0-4,3; 0.95 CI) and reoccurring administration of noradrenaline OR 4,6 (1,5-6,8; 0,95CI). The onset of gastrointestinal bleeding also has been registered more often in an observation group OR 3,5 (1,4-9,9; 0,95 CI) and gastrointestinal bleeding was associated with an increased level of intra-abdominal pressure γ 0,9, p<0,001, requirement of a feeding cancellation γ 0,75, p=0,04 and an administration of vasopressors γ-76, p<0,001. The relation was observed between duration of elemental nutrition and level of FABP II, γ 44, p=0,04. Administration of enteral feeding products with elemental composition has showed better tolerance and lower risk of complications associated with an enteral feeding, but most likely they are not suited for a prolonged use.
