СLINICAL SIGNIFICANCE OF COGNITIVE AND AFFECTIVE DISORDERS IN ELDERLY PATIENTS WITH CHRONIC HEART FAILURE

Abstract

Early identification of comorbid disorders, which can have a negative impact on the clinical status of patients with chronic heart failure, can contribute to improving the effectiveness of the therapy. The aim of the study was to study the clinical and functional characteristics and quality of life of elderly patients with chronic heart failure, depending on the presence of cognitive disorders and emotional disorders. 106 elderly patients with chronic heart failure II-IIIF (NYHA) were examined. Instrumental examination included standard electrocardiography, 24-hour Holter electrocardiography and echocardiographic parameters. The test included MMSE test, Beck test, Hamilton scale and Spielberger Hanin scales. 34 patients had mild cognitive deficits, 37 patients had vascular dementia, and 35 had anxiety-depressive disorders. Prognostically unfavorable frequent and group premature ventricular complexes are more reliably detected in patients with anxiety-depressive disorders. In these patients, the distance traveled through the 6-minute walk test was significantly lower and amounted to 115.9 ± 17.7 m (95% CI 109.8-121.9). Patients with dementia showed the most expressed reduction in indices according to scales of physical role functioning, emotional role functioning and mental health. The lowest indices of life quality according to the SF-36 and MLFHQ questionnaire were revealed in patients with anxiety and depressive disorders. Of particular importance is the violation of social functioning in connection with the violation of the integration of patients into society. The results of the investigation can be used in a comprehensive study of elderly patients with chronic heart failure to improve the effectiveness of personalized medical care.
