EMERGENCY SPECIALIZED AID for PATIENTs WITH CERVICAL CANCER
Abstract
In the article, on the basis of literary and own data, discussed in detail the problems of diagnosis, staging and treatment of cervical cancer at the stage of emergency this category of patients. The subject of this article is associated with the grim statistics of the prevalence of cervical cancer, as well as enough late detection of this nosological form, despite the available means of early diagnosis. Therapeutic-tactical errors in the management of patients with newly diagnosed tumors in non-specialized hospitals mainly due to inaccurate determination of the prevalence of unjustified rejection of radical surgical and combined treatment, which may be associated with late hospitalization, severe General condition of patients, adhering complications, lack of conditions for the treatment of cancer patients (the inability to perform round-the-clock urgent histological examination, the absence of chemioterapico and doctors radiologists and necessary for radiation treatment equipment), as well as the lack of generally accepted standards of emergency care in this category of patients. These constraints in General lead to a significant reduction in 5-year survival rate of gynecologic cancer patients operated in hospitals with a total network, compared with the rate in the cancer. The critical status of gynecological cancer patients mainly due to two main reasons: recurrence of the process at different times after the treatment or complications of surgery, radiation or chemotherapy. In these situations, the physician providing emergency patient care usually has to be limited to symptomatic therapy, aimed at correcting life-threatening problems (bleeding, intestinal obstruction, peritonitis), as well as the most painful for the patient manifestations of tumor (pain, intoxication). The article presents algorithms of diagnosis of cervical cancer in the primary treatment of non-oncological patient in the hospital. Described methods of conservative treatment of complications of tumor process. The recommended choice of volume of surgical treatment depending on the clinical situation. For informational purposes for physicians obego gynecological hospital provides specialized treatment regimens for cervical cancer. The article is primarily addressed to doctors and gynecologists hospitals for emergency and urgent care.
