CONTENT OF EMERGENCY SPECIALIZED SURGICAL CARE IN MODERN MILITARY CONFLICT
Abstract
The structure of sanitary losses at the stage of qualified surgical care (level 2) in a modern military conflict on a regional scale has been studied: the wounded make up 61.9%, including the seriously wounded — 15.2% of the total incoming flow. 7.6% of the wounded need specialized surgical care for urgent indications at the stage of medical evacuation of the 2nd level, and in general, 10.3% of the incoming flow of sanitary losses in advanced medical organizations of 2–3 levels. Specialized surgical care at the advanced stages of medical evacuation (level 2–3) was provided in three variants (models), all of them to one degree or another can be recommended depending on the current medical and tactical situation. In today’s large-scale or regional military conflict, it is impossible to implement a single scheme for providing specialized surgical care. The organization of surgical care should be determined by the conditions of the military conflict, the operational
and medical-tactical situation. Timely flexible monitoring of the situation and correct management decisions make it possible to quickly maneuver with forces and means, namely, to carry out targeted reinforcement of the stages of medical evacuation with the expected high level of sanitary losses by specialized specialists at the expense of doctors sent by military medical organizations of the district and the center. The most in-demand medical specialists after general surgeons to provide emergency specialized surgical care are orthopedic traumatologists and vascular surgeons. For a successful response, it is necessary to have an operational reserve of these in-demand medical specialists. Short-term courses for general surgeons in traumatology, angiotraumatology, and transfusiology are also a way to solve the problem of not always possible strengthening of the stages of medical evacuation.
