CREATION OF EMERGENCY DEPARTMENTS IN THE INTERDISTRICT CENTERS OF THE SAMARA REGION: THE FIRST RESULTS OF WORK
Abstract
In the organization of the work of emergency departments, the key tasks are the creation of a fundamentally new infrastructure and the implementation of processes that include modern approaches to life-threatening medicine and other emergency conditions, lean technologies and informatization of healthcare. As part of the implementation of the Federal project “Modernization of primary health care” in 2021–2022, 4 emergency departments were built on the territory of the Samara region on the basis of large inter-district centers. The construction was carried out using prefabricated modular structures with a usable area of 721 to 938 sq. m. The planned capacity of the departments is from 125 to 250 visits per shift. The total number of the attached population is 176 674 people with additional inter-district routing of another 82 348 people. An interim analysis of the results of the work of the branches in the period from April to October 2023 was carried out. The waiting time and the time of care for different categories of patients, the intensity of diagnostic equipment, the frequency of hospitalizations, redirections to regional centers, the number of telemedicine consultations and other criteria were evaluated. The average time to start providing care to patients with life-threatening conditions from the moment of admission was 95.0±22.7 seconds, the waiting time from the initial treatment to receiving the service in patients of the “yellow” and “green” categories was equal to 7.7±1.7 minutes and 14.0±3.2 minutes, respectively. There was an increase in the frequency of computed tomography by 22% per capita. In addition, the number of visits of the air ambulance service to these inter-district centers decreased by 9.3% and the number of telemedicine consultations increased by 15.2%. The immediate expected results of the activities of emergency departments will make a significant contribution to reducing hospital mortality and increasing public satisfaction.
