EVALUATION OF AVAILABILITY AND EFFICIENCY OF MEDICAL AID TO PATIENTS WITH ACUTE CORONARY SYNDROME AT THE STAGE OF PRE-ADMISSION

Abstract

Diseases of CVS nowadays present one of the major problems of health care system development. Mortality rate due to cardiac diseases takes the first place in all developed countries. To provide evaluation of availability and efficiency of medical aid to patients with acute coronary syndrome at the stage of pre-admission a study was undertaken on the base on the medical emergency station № 4 of Pushkin district, Saint-Petersburg. In the furtherance of this goal a complex of methods was used in the research: extracting from original medical records, expert assessment, graphic analysis method, content analysis and mathematical-statistical methods. As a part of the study the analysis and statistical processing of data of emergency calls in clinical records was provided: 84 patient records, form №110/у, from 01.01.2014 to 31.12.2015. Also 43 specially designed questionnaires were developed to extract data from original medical records of patients with acute coronary syndrome. The use of thrombolytic therapy in Pushkin district of Saint-Petersburg is reasonable considering remote location of the nearest Heart Centres and in-patient clinics, bad transport accessibility and overburdening of the primary healthcare agencies in the district. The assessment of first medical aid to patients with acute coronary syndrome showed that accessibility of first medical aid does not fully correspond to proposed specification of time-on-target of emergency ambulance.
This influences pre-hospital mortality and further prognosis of the patients with acute coronary syndrome. The research of patients who received thrombolytic therapy in presence of acute coronary syndrome at the stage of pre-admission showed the efficiency of the therapy. More than a half of all the patients receive this type of medical aid.
