Abstract

Primary health care (PSMP) is the basis of the system of medical care, as a stage of primary contact between the health system and the population. One of the main constituent elements of ensuring quality and accessibility of medical care is staffing of primary health care organizations with qualified medical personnel. As part of the reform of the PHC organization, in order to preserve and replenish human resources in medical organizations, it is necessary to provide for the improvement of remuneration of labor by developing indicators and criteria for the effectiveness and quality of labor, that fully reflect the results of individual labor activity of medical workers, and the salary formation system should become transparent and understandable for employees. The article presents the main problems of the remuneration system in primary health care medical organizations, an assessment of the existing remuneration system and the salary structure of medical workers is carried out on the example of medical organizations providing primary health care in St. Petersburg. As a result of the analyses of the survey of doctors and nurses of these medical organizations, it was revealed that the vast majority of them are not familiar with the structure of their wages, are not satisfied with its amount, are not familiar with the indicators of evaluating the result of their work, believe that personal labor costs exceed the salary level, are not sure that wages are calculated clearly and transparently. In conclusion, the authors state that in order to motivate medical workers to perform their job duties efficiently and effectively, it is necessary to revise the remuneration system using the labor rationing system.
