Abstract

The priority of the health of children is one of the basic principles of domestic health care. Improvement of activities in the field of child health protection, of availability and quality of children medical care is an integral part of a social state. The provision of primary health care to children in rural health care institutions is associated with many problems that cannot be solved without a patient oriented approach in assessing the state of modern system and choosing the most effective complex measures of futher development. In order to evaluate the work of the children's neurological service in the local polyclinic attached to the central regional hospital (CRH), 250 parents of children aged 0-17 years old were surveyed. The study was conducted on the basis of a children's local polyclinic of the Central Clinical Hospital of the Semikarakorsky district of the Rostov region with the help of a random sampling method. It was established that more than a quarter of parents experience difficulties in transport accessibility. More than a third of respondents are not satisfied with the working schedule of the neurologist and more than a quarter of them couldn' make an appointment with a clinic neurologist mainly due to the absence of a doctor (47.2%). Most of the respondents (58.1%) could not manage to be admitted by a neurologist at the appointed time for such reasons as a live queue (58.8%), a prolong absence of a doctor in the site (30.3%) and a heavy workload of the neurologist in the CRH (10, 9%). The waiting period was more than 1 hour in 8.1% of respondents who had made the appointment beforehand, and those without it in 46.3%. Massage and physiotherapy exercises were the least accessible medical procedures prescribed by a neurologist, and among the instrumental types of research are neurosonography and Doppler ultrasound. The low availability of neurologist consultations, examinations and medical procedures contributed to the fact that parents had to pay for medical services that otherwise are free from charge. At the same time, more than half of the parents (53.6%) were not satisfied with the quality of the organization of neurological care and rated it at 3.2 ± 0.1 points. The main reasons for dissatisfaction were long awaitance (29.7%), incompetence and insufficient qualification of the doctor (27.6%), as well as low diagnostic possibilities for examination (21.3%).
