Abstract

Obliterating bronchiolitis is a severe obstructive disease of the “small airways” with the development of “exudative granulomatous reaction and fibrosis”, leads to irreversible changes in the walls of bronchioles, to concentric narrowing and bronchial obliteration, most pronounced in the terminal areas, persistent bronchial obstruction and chronic inflammation, hyperinflation and emphysema and areas of atelectatic fibrosis. Chronic inflammation, bronchiectasis is formed in the large bronchi. The disease is polyetiological, while it can also be a morphological substrate of other diseases. The introduction of computed tomography into clinical practice has made it possible to conduct a fine structural analysis of the bronchial tree up to the bronchioles and improve the possibility of diagnosing this pathology in pediatrics. On the example of the presented clinical observation of an 11-year-old patient with a diagnosis of bronchiolitis obliterans, the role of spiral computed tomography in visualization and the possibility of structural analysis of the bronchial tree, anamnestic data and endoscopic examination in establishing the diagnosis and cause of the pathology was noted, which made it possible to reduce the use of medications and improve quality of life, frequency of exacerbations and lung function tests.
