TESTICULAR TORSION IN ADULTS: HOSPITAL EXPERIENCE

Abstract

Background. Testicular torsion occupies a leading position among acute diseases of the scrotal organs. The severity torsion is the main determinant of testicular tissue survival, depending
on the duration and degree rotation. This urological situation is dangerous, as prolonged diagnosis of the emergency condition and untimely medical care can lead to testicular necrosis. Methods. We analyzed the medical histories of patients from 2020–2023 who were brought by ambulance to Mariinsky Hospital with scrotal organ diseases. We selected patients from the hospital database using ICD-10 codes. Of the 395 patients with symptoms of scrotal disease, testicular torsion was diagnosed in 33 patients. Results. Two patients with testicular underwent manual detorsion in the emergency room. In 4 patients, the pain resolved on its own. The remaining men underwent revision of the scrotal organs, in which 23 patients underwent orchiopexy, orchiectomy was performed in only 4. In patients aged 18–25 years the postoperative period passed without special complications, in older patients there was a temporary pain syndrome, which was relieved by taking non-steroidal anti-inflammatory drugs, in some patients the temporary rose to 37,5 °C and decreased due to antibacterial therapy. At the control examination of the group of patients no late complications were revealed. Conclusion. Difficulty in diagnosis and not typical clinical picture often lead to incorrect diagnosis and prescription of wrong treatment with subsequent unfavorable outcomes. The need to raise public awareness of acute diseases of the scrotal organs plays a key role in ensuring timely and adequate treatment.
