A CLINICAL CASE OF SEPSIS IN THE FORM OF SEPTICEMIA IN A NEW CORONAVIRUS INFECTION OF COVID-19 WITH PRIMARY AND SECONDARY FORMS OF INFECTIVE ENDOCARDITIS

Abstract

This article presents a case of a rare combination of viral pneumonia caused by a novel COVID-19 coronavirus infection with sepsis in the form of septicemia and with the presence of primary and secondary forms of infective endocarditis in an 89-year-old woman. The patient was admitted in serious condition with a verified new COVID-19 coronavirus infection. After staying in the hospital for 24 hours, the woman died due to the progression of acute heart failure, so the main diagnostic measures were not performed in full (due to the severity of the patient’s condition and short stay in the hospital). The sectional examination revealed: 1) viral pneumonia caused by COVID-19 in the fibrosis phase with acute interstitial infiltration due to neutrophilic granulocytes; 2) sepsis in the form of septicopoiesis (acute microabscesses in the heart, kidneys, brain, thyroid gland, and fibrinous purulent pericarditis). In addition, infective endocarditis of the aortic and tricuspid valves was detected. During pathomorphologic examination, it was found that the patient had 2 forms of infective endocarditis: primary and secondary (there is evidence of this in the paper and in the conclusions). Taking into account the above mentioned, the paper displays morphologic differences between the two forms of endocarditis and peculiarities of diagnoses rubrication in the simultaneous presence of both forms of endocarditis and sepsis in the form of septicopemia.
