SINGLE-PHOTON EMISSION COMPUTED TOMOGRAPHY WITH AUTOLEUKOCYTES LABELED WITH 99MTC-HMPAO IN THE DIAGNOSIS OF INFLAMMATORY HEART DISEASES (MYOCARDITIS AND ENDOCARDITIS)
Abstract
Introduction. Inflammatory diseases of the cardiovascular system are lifethreatening conditions with high morbidity and mortality. Heart diseases can be caused by both bacterial and viral infections. Medical imaging plays a key role in the diagnosis of this cardiac pathology. Pathophysiological inflammatory processes at the cellular or molecular level can now be determined using new radionuclide diagnostic methods: PET/CT with 18F-FDG and SPECT/CT with labeled leukocytes. Purpose of the study. To evaluate the capabilities of single-photon emission computed tomography with autoleukocytes labeled with 99mTc-HMPAO (hereinafter SPECT with 99mTc-HMPAO-WBC) in the diagnosis of inflammatory heart diseases, in particular, myocarditis and infective endocarditis. Materials and methods. Fifteen 99mTc-HMPAO-WBC SPECT scans were analyzed in patients with suspected cardiac infection. Results. SPECT with 99mTc-HMPAO-WBC has low sensitivity in diagnosing myocarditis, but demonstrates high sensitivity and specificity in detecting endocarditis, both prosthetic valve (PVE) and native valve (ENV). Conclusions. SPECT with 99mTc-HMPAO-WBC is an effective method for non-invasive diagnosis of infectious heart diseases, especially endocarditis of prosthetic valves.
