Abstract

Background. The identification of previously underestimated links between abdominal obesity and indicators of carbohydrate metabolism, chronic sluggish inflammation and the structural and functional state of the vascular wall will expand the current understanding of cardiovascular risk factors in young men with abdominal obesity. Aims. To study the relationship between indicators of carbohydrate metabolism and pro-inflammatory changes, the structural and functional state of the vascular wall, and biomarkers of endothelial dysfunction in young men aged 18‒21 years with abdominal obesity. Materials and methods. 221 young men with abdominal obesity were examined. Biomarkers of endothelial dysfunction have been studied; indicators of blood lipid spectrum and carbohydrate metabolism; highly sensitive C-reactive protein; the results of functional tests to determine endothelial dysfunction and the data of the contour analysis of the pulse wave. Results. An inverse correlation was established between glycated hemoglobin and endothelium-dependent (r = –0.29; p < 0.05) and endotheliumindependent vasodilatation (r = –0.30; p < 0.05). A positive correlation was found between glycated hemoglobin and the augmentation index (r = 0.35; p < 0.05), age of the vascular wall (r = 0.14; p < 0.05), and the level of circulating endothelial cells (r = 0.55; p < 0.05), P-selectin (r = 0.48; p < 0.05), interleukin-6 (r = 0.27; p < 0.05) and highly sensitive C-reactive protein (r = 0.76). Other indicators of carbohydrate metabolism showed a similar relationship. Conclusions. In boys aged 18–21 years with abdominal obesity, the deterioration of carbohydrate metabolism is associated with pro-inflammatory reactions, impaired functional state of the vascular wall, and the development of endothelial dysfunction.
